Liability Waiver & Assumption of Risk

1) Assumption of Inherent Risks

Soccer involves physical activity and inherent risks including collisions, falls, contact with equipment, and environmental conditions. The undersigned understands and voluntarily assumes these risks for themself and/or the minor participant.

2) Health Disclosure & Preparedness

The undersigned agrees to disclose relevant medical conditions and provide necessary medications (e.g., clearly labeled EpiPens/inhalers) to the Site Lead at check-in. All Site Leads are CPR/First Aid certified.

3) Emergency Medical Authorization

PTP staff may obtain emergency medical care deemed reasonably necessary. The undersigned agrees to be financially responsible for associated costs.

4) Release & Waiver of Liability

To the fullest extent permitted by Pennsylvania law, the undersigned releases and holds harmless PTP-PLAYERSTEACHINGPLAYERS LLC and its owners, officers, employees, contractors, volunteers, and agents from claims, demands, or causes of action arising from participation in PTP events, except where caused by PTP’s gross negligence or willful misconduct.

5) Indemnification

The undersigned agrees to defend, indemnify, and hold harmless PTP and its affiliates from claims arising out of the participant’s acts, breach of PTP policies, or violation of law.

6) Conduct; Policies Incorporated

The undersigned agrees to comply with PTP’s Code of Conduct, Media Release, and posted safety rules. Failure to comply may result in removal without refund, consistent with the Refund Policy.

7) Venue; Severability

Pennsylvania law governs; exclusive venue lies in courts located in Pennsylvania. If any provision is held invalid, the remainder stays in effect.
